Viết plan.md với acceptance criteria và boundaries.

Tạo flat task-NN có ownership, dependencies và proof command.
05 / C2
Đưa material review findings cho user quyết định.
Output
`plan.md` đã review cùng flat `task-NN-*.md`, sẵn sàng cho invocation develop mới.
Tránh
Không dùng spec như tài liệu trang trí sau khi code đã đoán xong. Spec phải constrain implementation.
